How can I obtain an extruded wall face plate?

I need a wall face plate, but not just flat like usual, rather extruded out from the wall about an inch.

The problem is that the hole in the wall is very small, and so the cable will bend before going into the hole, so I need a plate that will have enough space to allow that to happen.

I happen to need one that is Ethernet/coax duo, but any information about any type of plate matching the description would be helpful.

You can get the plate at hardware stores. Extend the box with something like this http://www.homecontrols.com/Arlington-Box-Extender-AABES1 They have them at hardware stores too.

Just get a shallow one-gang Wiremold box and mount it over the cable access, then use a standard flat plate on it with the jack arrangement you need. These are available in any hardware, building supply or electrical supply store. The backplate screws to the wall (after you knock out whichever opening you need for the cable in it) and then the cover box (similar to the "extrusion" you are describing) covers it with room for the cable and devices inside. I installed thousands of these when I was an installation electrician, especially when renovating old schools and offices with data jacks. Here is what it looks like.
